Supporting Those Exiting the Prison, Care Home or Military Systems Who Are at A Crossroad.

360-degree referral service available to male, female or young offenders (YO’s 18-21 years old), with a core objective to change their life and resettle back into a community.


A vital care model consisting of 8-10 single bedrooms with: Wardrobes, chest of drawers, three bathrooms, communal large kitchen, meeting room for delivery of support packages.  Care package to include: Referral, initial assessment, risk and needs assessment, allocation of address, gate pick up, 13-20-week programme of support and foundation building, move on support to:

  • Make communities safer
  • Reduce reoffending
  • Improve the chance of resettlement

As a rule, offenders who are in the last 9 weeks of their sentence are referred to a member of our referrals team. A member of our referrals team will be contactable through our referral programme and will arrange to meet the service user whilst they are still in the women’s prison estate and explain the service, approximately 1 month before release.

On the day of release, our referral team will meet the service user at the prison gate, where they will be greeted with a “Life Bag” containing the following vital information and products:

New Bedding
Bank Account

Service users will be escorted to appointments and other support agencies if required. At the end of the 12 – 20 week supported care programme, the support team will complete onward referrals with the service user / resident to ensure that more long-term support is put in place.
